In the State of Indiana:
- You must complete an Indiana Operator’s Crash Report (See Forms) within 10 days of an auto accident to avoid suspension of your license.
- An Employer’s First Report of Injury (See Forms) must be filed within 7 days of the occurrence or the employer’s knowledge of injury or death to avoid a $50 civil penalty.
- For your protection, you must notify us immediately if you receive a lawsuit. You have a limited number of days from the receipt of a summons to respond without penalty.
